Adler cites this from Synesius, Letter 113 (254b), where in fact it is presented as already proverbial (see the translation of A. Fitzgerald at web address 1).
The paroemiographers who include it do so, as Synesius does, with a kai/ ('even') omitted here; and they gloss it as applying to the aged or unfortunate who are still in good shape.
