[1] = Synagoge pi516, Photius pi3981 Theodoridis; cf. Hesychius pi2716. Adler dubs the entry 'gl.Hom'. The headword in this form (nominative singular), erroneously given in the online headword, is scarcely attested outside lexicography; but the vocative form poikilomh=ta and the accusative poikilomh/thn, do frequently occur in Homer as formulaic epithets for Odysseus and others. In his edition of Hesychius Latte calls attention to Iliad 11.482 (accusative); Theodoridis adds Odyssey 3.163 (likewise).
[2] A rarer form of essentially the same word, following a different strategy of noun-formation: the primary headword uses an alpha-declension agentive ending, while this form preserves the consonant-declension ending of the second of the constituent nouns of the compound mh=tis ('wits', 'intelligence'). Again, scarcely attested in this form (nominative singular); see e.g. Nonnus, Dionysiaca 8.125, Greek Anthology 9.670.5 (author unknown); unlike the primary headword, however, this is also rare in other forms (e.g. Sophocles fr. 592 Radt, nominative plural).
