Makeup Request for CS 101 Lab Tests

The next makeup for Lab Test 3 will be Tuesday, November 24, at 2 pm in RGAN 103.

• Remember that makeups for lab tests are ONLY allowed for the excuses accepted by UK, i.e., a death in the immediate family, a serious illness, an academic or athletic trip for the University or a religious holiday. See the syllabus for details.
• Fill out the form below - incomplete forms may be ignored! NOTE: you will not be contacted by anyone just because you filled out this form!
• If you CAN come to the time and place above, all you have to do is come to the makeup and bring your documentation for your excuse. We need a PAPER copy that we can keep for our files, with your name and section written on it. PLEASE be on time - the time penalties for makeup tests are the same as for the scheduled ones.
• If you CANNOT come to this time and place, fill out the form anyway and use email to Dr. Keen (keen@cs.uky.edu) to arrange another time. It is NOT guaranteed that another time can be arranged!
• It is assumed if you make the request and do not contact anyone by email, you WILL come to the time and place above. If you miss this makeup opportunity, you will NOT be allowed to request another opportunity for that lab test.
